What Is the Internal Medicine Exam?
The Internal Medicine End of Rotation™ exam is administered by PA programs at the end of the clinical rotation. It contains 120 multiple-choice questions — 100 scored and 20 unscored pretest items — with a two-hour time limit. Questions span the full blueprint, including task areas like diagnosis, diagnostic studies, clinical therapeutics, and clinical intervention.
Updated for the 2026 PAEA Blueprint
PAEA publishes new versions of the EOR exams on July 27, 2026, and the Internal Medicine EOR blueprint changes with them. Critical care disappears as a standalone content area (its topics are redistributed into the organ systems), psychiatric/behavioral health is added at 5%, and endocrine, neurologic, and renal each rise to 10%. Cardiovascular holds at 20% and remains the largest area, while musculoskeletal takes the biggest cut, falling to 8%. Diagnostic Studies jumps from 10% to 14%.
