The Surgery End of Rotation (EOR) exam is the odd one out. It was already rebuilt in 2024 — ahead of the other six — and it was the exam that introduced the new content-area and task-area structure the rest are now adopting in 2026. What is on the Surgery EOR exam?

The Surgery EOR exam is 120 multiple-choice questions with a two-hour limit and a ten-minute break between sections. Only 100 of the 120 are scored — the remaining 20 are unscored pretest items, and you cannot tell which is which.

Surgery was updated in September 2024 and is not part of the July 2026 release. Its blueprint is already current.

Perioperative setting targets

Surgery is the only EOR exam with a third dimension. Every item is also coded to a perioperative setting, and the exam is built to these targets:

What this means for how you study

Postoperative care is 40% of the exam. This is the most under-appreciated fact about the Surgery EOR. Students study anatomy and operative technique; the exam asks about the patient on postoperative day three with a fever. Know the postoperative fever timeline, wound complications, anastomotic leak, ileus versus obstruction, DVT prophylaxis, and postoperative pain management.

GI is still the largest content area at 17%, but PAEA deliberately reduced it in the 2024 rebuild. It used to dominate the exam far more heavily.

Three content areas are relatively new: breast surgery (10%), trauma/acute care (8%), and pain medicine/anesthesia (7%) — 25% of the exam between them. Older surgery review resources barely cover these.

Professional Practice replaced Health Maintenance as a task area on this exam — informed consent, surgical timeouts, disclosure of complications.

How to study for the Surgery EOR

Study the ward, not the operating room. Forty percent of the exam is postoperative. You will be asked about the complication, not the incision. Postop fever (the classic five Ws), ileus versus small bowel obstruction, anastomotic leak, wound dehiscence, atelectasis, DVT.

Learn preoperative risk assessment. Preoperative is another 35%. Cardiac risk stratification before non-cardiac surgery, which medications to hold and when, NPO guidelines, antibiotic prophylaxis timing.

Do not skip the three newer content areas. Breast, trauma/acute care, and pain/anesthesia total 25% of the exam and are thin in most older resources.

Do questions early, not at the end. Questions are how you find out what you do not know; reading is how you fix it.

Is the Surgery EOR changing in July 2026?
No. Surgery was already updated in September 2024 and is not part of the July 2026 release. Its blueprint is current.

What are the perioperative setting targets?
Surgery is the only EOR exam that codes every item to a perioperative setting: preoperative (35%), intraoperative (25%), and postoperative (40%). Postoperative care is the largest single slice of the exam.

Will this help with the PANCE?
Yes. The PAEA Surgery blueprint was developed with explicit reference to the NCCPA PANCE blueprint.
