The exam formerly known as Women’s Health has been renamed and completely rebuilt. It is now the Gynecologic, Sexual, and Reproductive Health (GSR) End of Rotation exam, and the 2026 update is the most thorough reorganization of any of the seven EOR exams. What is on the GSR / Women’s Health EOR exam?
The GSR EOR exam is 120 multiple-choice questions with a two-hour limit and a ten-minute break between sections. Only 100 of the 120 are scored — the remaining 20 are unscored pretest items, and you cannot tell which is which.
The 2026 GSR blueprint update — a complete rebuild
PAEA publishes updated End of Rotation exams on July 27, 2026. Legacy versions remain available through July 2027, so ask your clinical coordinator which blueprint your program is using.
This is not a reweighting — it is a restructuring. The legacy blueprint split the exam into Gynecology (menstruation, infections, neoplasms, breast, structural) and Obstetrics (prenatal care, pregnancy complications, labor & delivery). The updated blueprint discards that split entirely and reorganizes around anatomy and areas of care.

What this means for how you study
Three brand-new content areas account for 26% of the exam. Fertility management (10%), Sexual health & development (10%), and Psychiatric/behavioral health (6%) did not exist as discrete areas before. That is roughly thirty scored questions on material an older resource will not have covered properly.
Topics were redistributed, not deleted. Menstruation — a 15% category on the legacy blueprint — now lives inside Sexual health and development. Neoplasms and structural abnormalities have been spread across the anatomical areas (uterine/cervix, ovary/adnexa, vulva/vaginal, breast). If you study from a resource organized around the old gyn/OB split, you will have gaps and you will not know where they are.
Pregnancy is still the single biggest area at 20%, and with Labor & delivery (9%) and Postpartum care (8%), obstetric content is 37% of the exam.
Fertility management at 10% is a real shift. Contraception, infertility workup, assisted reproduction, PCOS. Ten percent of the exam.
Professional Practice is new at 8% — tied with Pediatrics for the highest of any exam. Consent, confidentiality, adolescent care, and reproductive decision-making.

How to study for the GSR EOR
Obstetrics is 37% of the exam. Prenatal care (20%), labor and delivery (9%), and postpartum (8%). Know the prenatal visit schedule, the screening tests and when they happen, the hypertensive disorders of pregnancy, and the postpartum complications. This is where the questions are.
Do not skip fertility management. Contraception method selection and contraindications, infertility workup, PCOS. It is 10% and it is new.
Know the contraindications. Combined oral contraceptives in a migraine-with-aura patient, in a smoker over 35, in someone with a history of VTE. This is exactly the kind of discrimination the exam is built on.
Do questions early, not at the end. Questions are how you find out what you do not know; reading is how you fix it.
