He reports 45 minutes of substernal chest pressure radiating to the left arm with diaphoresis and nausea. HR is 96/min, BP is 138/82 mm Hg, and SaO₂ is 98% on room air. ECG demonstrates 3-mm ST-segment elevations in leads V2 through V5.

This patient has an acute anterior STEMI. When timely PCI is available, immediate reperfusion with PCI is preferred. Do not delay activation of the catheterization laboratory while waiting for serial biomarkers or additional imaging.

Serial troponins confirm myocardial injury but should not delay reperfusion in a patient with diagnostic ECG findings. Outpatient stress testing is inappropriate in an active STEMI. A chest radiograph may evaluate alternative diagnoses but is not the next priority. Fibrinolysis is reserved for situations in which timely PCI is unavailable or cannot be performed within the recommended treatment window.
