1. Start with the amount of CME you actually need

Laboratory testing shows WBC 19,000/mm³ and lactate 4.6 mmol/L. You administer broad-spectrum IV antibiotics and 30 mL/kg of crystalloid. His MAP remains 58 mm Hg.

Answer and Explanation
B. Begin a norepinephrine infusion.
This patient has septic shock with persistent hypotension after adequate initial fluid resuscitation. Norepinephrine is the first-line vasopressor to restore vascular tone and maintain MAP, generally targeting at least 65 mm Hg.
Furosemide would worsen intravascular depletion. Additional antibiotics should not delay hemodynamic support. Dopamine is not preferred because of its higher risk of tachyarrhythmias. CT angiography of the chest does not address the immediate shock state.
This is the type of next-step management question that should be part of your PANRE review routine: identify the syndrome, assess the immediate threat, and select the intervention that changes short-term mortality or stabilization.
