A chest radiograph is the correct initial diagnostic test for suspected community-acquired pneumonia in a patient with fever, productive cough, pleuritic pain, focal lung findings, and hypoxemia. The radiograph can identify an infiltrate and help assess the extent of disease.

CT pulmonary angiography and D-dimer testing are not first-line choices when the clinical presentation is more consistent with pneumonia than pulmonary embolism. Anticoagulation should not be started without an appropriate indication. Bronchoscopy is reserved for selected cases, such as persistent, unexplained, or atypical pulmonary disease.
