The course is designed for practicing PAs preparing for either the traditional PANRE or PANRE-LA. The 100 AAPA Category 1 credits apply to the PANRE Clinical Review Module specifically. They do not automatically apply to every other CME or pharmacology product.

You must confirm your eligibility and application window through your NCCPA portal. PAs seeking to regain certification after expiration or revocation are generally required to use the traditional PANRE rather than PANRE-LA.

Who May Prefer PANRE-LA?

PANRE-LA may fit your practice if you perform better with spaced repetition and regular feedback rather than one high-stakes testing date.

The format can be practical for a PA who:

• Has a demanding clinical schedule
• Wants to study in smaller sessions
• Prefers using references to confirm clinical reasoning
• Benefits from immediate explanations after each question
• Wants performance feedback over time
• Is comfortable maintaining a consistent quarterly schedule

The longitudinal model does not eliminate the need for preparation. You still need broad knowledge across cardiovascular, pulmonary, GI, infectious disease, neurologic, psychiatric, reproductive, renal, endocrine, musculoskeletal, dermatologic, hematologic, EENT, genitourinary, and professional practice topics.

A common mistake is treating PANRE-LA as an open-book replacement for studying. References can help you verify a difficult point, but they do not replace pattern recognition, diagnostic prioritization, or familiarity with first-line management.

Your patient is a 68-year-old man with COPD who presents with fever, productive cough, and dyspnea. He is confused. Vital signs are BP 86/50 mm Hg, HR 118/min, RR 30/min, and SaO₂ 88% on room air. Chest radiography demonstrates a right lower-lobe infiltrate. After receiving 30 mL/kg of IV crystalloid, his BP remains 84/48 mm Hg. Which of the following is the most appropriate next step?

A. Administer IV furosemide
B. Begin a norepinephrine infusion
C. Start oral prednisone and discharge
D. Order an outpatient CT chest
E. Administer IV propranolol for tachycardia

Correct answer: B. Begin a norepinephrine infusion

This patient has presumed septic shock from community-acquired pneumonia with persistent hypotension after an appropriate initial crystalloid bolus. Norepinephrine is the first-line vasopressor when hypotension persists despite fluid resuscitation.

Furosemide may worsen intravascular depletion. Prednisone and discharge are inappropriate in a patient with shock and altered mental status. CT chest delays resuscitation and does not change the immediate management priority. Propranolol can worsen hypotension and is not indicated for compensatory tachycardia.

This is the type of management-focused reasoning tested across PANRE and PANRE-LA content areas. Practice should include both diagnosis and the next best step, with attention to hemodynamics, medication effects, contraindications, and resource utilization.
