He has no history of contrast allergy or renal disease.

Which is the most appropriate next diagnostic step?

A. D-dimer assay
B. CT pulmonary angiography
C. Bilateral lower-extremity venous ultrasound only
D. Repeat examination in four hours
E. Exercise stress testing

Answer and explanation

B. CT pulmonary angiography is correct. This patient has a high pretest probability for acute pulmonary embolism based on recent surgery, tachycardia, hypoxemia, pleuritic chest pain, and unilateral leg findings. He is hemodynamically stable and has no contraindication to iodinated contrast, making CT pulmonary angiography the appropriate diagnostic study.

A is incorrect because D-dimer is most useful when the pretest probability is low or intermediate. In a postoperative patient with a high clinical suspicion, a positive result is expected and does not efficiently establish the diagnosis.

C is incorrect because lower-extremity ultrasound may identify DVT but does not directly evaluate the pulmonary vasculature. It may be useful when CT contrast is contraindicated, but it is not the preferred next test here.

D is incorrect because delaying evaluation risks progression of an untreated PE.

E is incorrect because stress testing does not evaluate the suspected acute thromboembolic process.
