If you are a PA planning for the PANRE, your CME strategy should address two separate goals: exam preparation and NCCPA certification maintenance. The key point is specific: the 100 hours of AAPA Category 1 CME apply to the PANRE Clinical Review Module of the PANRE Review Course. They do not automatically apply to every CME Review Courses product or to the separate Pharmacology course.
NCCPA certification maintenance requires 100 CME credits during each two-year CME cycle. At least 50 credits must be Category 1 CME. The remaining credits may be Category 1, Category 2, or a combination of both.

AAPA Category 1 CME is not the same as informal professional reading or general clinical activity. Category 1 activities are formally planned, educationally relevant, and reviewed according to accreditation standards.
Examples of Category 2 activity may include reading medical literature, informal learning, or other practice-related education that does not carry formal Category 1 approval. These activities may still have educational value, but they do not satisfy the Category 1 portion of your cycle.
A formal PANRE review course provides a more organized path. The course is built around exam-relevant clinical content, measurable learning objectives, practice questions, and post-test requirements. To claim credit, you must complete the required activity components and claim only the amount supported by your participation.
A common CME problem is fragmentation. You may complete a few credits from a conference, several online activities, and additional Category 2 education throughout the year. That approach can work, but it requires consistent tracking and attention to the Category 1 minimum.

The course is primarily designed for practicing physician assistants. Other clinicians may find the clinical content useful, but credit treatment can differ.
Nurse practitioners: Some state boards, employers, or certification organizations may accept AAPA Category 1 CME. Acceptance varies by state. NPs should verify requirements directly with their state board of nursing and certifying organization before purchasing.
Physicians: Physicians may find the content clinically useful, but AAPA Category 1 CME generally does not function as Category 1 CME for physicians. Physician acceptance may be limited to Category 2 or subject to the rules of the physician’s certifying organization and state licensing board.

Which medication should be administered first to reduce aortic wall shear stress?
A. IV nitroprusside
B. IV esmolol
C. IV heparin
D. Oral amlodipine
E. Aspirin
The correct answer is B. IV esmolol.
In suspected acute aortic dissection, begin short-acting IV beta-blockade to reduce HR, contractility, and the rate of pressure change across the aortic wall. Esmolol is useful because it can be rapidly titrated. Labetalol is another appropriate option.
Nitroprusside should not be used before beta-blockade because reflex tachycardia can increase aortic shear stress. Heparin and aspirin may worsen bleeding if dissection is present. Oral amlodipine is too slow and is not appropriate for immediate control of this hypertensive emergency.
