Reading can create familiarity, but familiarity is not the same as retrieval. On the examination, you must identify the relevant diagnosis, select the next step, recognize contraindications, and distinguish similar answer choices under time pressure.

Do not judge progress only by your percentage correct. Review the reasoning behind each response. A correct answer based on an incorrect rationale still represents a knowledge gap.

This patient has a high clinical suspicion for pulmonary embolism based on recent major surgery, tachycardia, hypoxemia, pleuritic chest pain, and unilateral calf symptoms. Because she is stable and has a high pretest probability, proceed directly to definitive imaging with CT pulmonary angiography. A D-dimer is most useful when the pretest probability is low or intermediate; in this postoperative patient, it is likely to be elevated and would not efficiently rule out PE. Antibiotics and albuterol do not address the primary concern.
